I understand that my child is enrolled in a catechetical program whose sole purpose is to assist my child in developing a relationship with Jesus.

I understand that the sacraments (Baptism, Penance, Communion, and Confirmation) are among the most important moments of my child's life in which my child will encounter Jesus.

I understand that my child will receive instruction for those sacraments and must meet all the minimum requirements of the Archdiocese of New York (especially with regard to attendance, homework assignments, group projects, testing, conduct, community service, and retreats, etc.) before my child can be considered successful and promoted to the next level of catechesis.

I understand that my child's catechesis incurs fees that must be paid before the end of each year.  I understand that those fees, currently $150 per year per child, cover all classroom materials, sacramental supplies, retreat costs, and church donation).  I also understand that those fees are  partially refundable after consultation with the pastor.

I understand that in case of an accident or illness involving my child, a representative of the religious education program will call me.   If I am unable to be reached, I hereby authorize him or her to make whatever arrangement seems necessary including but not limited to calling Emergency Medical Services to transport my child to the nearest emergency room for medical treatment.  I agree to assume financial responsibility for any diagnosis, treatment, and or medication deemed necessary.

I understand that to the fullest extent permitted by law, I or my child shall defend, protect, indemnify and hold harmless the Church of the Sacred Heart or the Church of St. Francis of Assisi and any of its partners, clergy, religious, directors, officers, agents, family members, helpers, organizational members, volunteers, employees and associates thereof from and against all damages, losses, expenses, and claims arising from my negligence or fault or which arise out of the enrollment of my child in the religious education programs of the Church of the Sacred Heart or the Church of St. Francis of Assisi.
